Our Team

RN Case Managers
Our nurses educate and guide patients and families in symptom management and safety techniques in the patient’s home. Nurses are available for guidance or visits 24 hours a day. An inter-disciplinary team is coordinated by the RN Case Manager to develop and implement the patient’s individualized plan of care.

CNA Home Health Aides
At the direction of the care plan and patient’s wishes, our CNAs are available 1 to 5 times a week to care for daily needs ranging from bathing to light housekeeping.

Medical Social Worker
Our master’s level social worker assists families with community resources and advanced directives such as Healthcare POA, Durable POA, and Living Will. The social worker supports and counsels families on issues surrounding life-limiting illness.

Chaplain services will be offered to provide spiritual nourishment and care to patient and family during this difficult journey.

Bereavement Coordinator
Bereavement services are offered to families who have experienced the loss of a loved one. Patient families may choose to arrange a yearly memorial service for their loved one, support group meetings, periodic mailings and/or phone calls to assist with transition after loss.

Our team of volunteers will be glad to build a ramp for you or sit and watch a ballgame with you. This special group of people will show you, as much as anyone, how Hands of Mercy becomes a part of your family.
